S-adenosylmethionine (SAM-e):is a naturally occurring compound critical to the maintenance of cartilage and the manufacture of brain compounds such as neurotransmitters. Some people, especially the elderly, have low levels of S-adenosylmethionine in their bodies. Scientists know that certain groups of people, including most elderly, have low levels of Sam-e.
Popular in Europe where it has been distributed by doctors as a prescription medicine for over 20 years. This product is now "Hot" in the USA. What is SAM-e? It's short for S-adenosylmethionine. It's produced naturally by the body. It's not an herb or what you would consider a regular supplement because it's so active in the body, the energy compound of the body ATP, adenosine triphosphate. It's a very exciting supplement. It's been used in Europe for over 20 years. There's so much research on it to do so many great things to the body. So why do we need a supplement, since it's already in the body? Interestingly enough we do produce it but not enough. Studies have shown that as we age, we produce less and less SAM-e. Another thing is that most of our diets are not perfect, because you need folic acid and vitamins for the body to produce and use SAM-e. Would SAM-e be beneficial in any type of arthritis disease? That's a great question. There needs to be distinction among types of arthritis. There's rheumatoid arthritis. There's osteoarthritis which is a cartilage breakdown in a localized area like knee joint or hip joint. SAM-e's clinical studies -- there have been over 75 clinical studies. Most of the research and the clinical back-up for using SAM-e for joint health is based on osteoarthritis. Theoretically, there's been a bit of research here and there that we may be able to use SAM-e for the other types of arthritis also. Right now it's showing benefit in osteoarthritis only. Is Osteoarthritis the type that comes with age? Wear and tear can affect younger people depending on how well they take care of their joints. Someone like an athlete may want to do preventative measures because there's been research showing that SAM-e may help prevent and regrow cartilage loss.
How do you know if your cartilage is messed up? See your doctor. Then if you have constant pain in a localized area, chances are there can be some sort of arthritis. If it's constant reinjuring of the same area, those things contribute to breaking down the cartilage in your joints.
How does a doctor determine if you have arthritis? Osteoarthritis, through X-rays. You can actually see the cartilage deteriorated.
Is SAM-e it the same as glucosamine?
No they're different.
Are the results the same?
They are getting results from glucosamine but the jury is not out scientifically like it is with SAM-e. The jury is still out if the glucosamine is going to be as useful as the SAM-e. Does cartilage regrow? Why or why not?
There have been some studies that have shown that SAM-e can actually help increase the chondrocytes in the bone, which is where the cartilage is produced.
What are SAM-e's effects when it is taken in larger quantities than your body itself produces? Right now there's very little side effects, especially as compared to what people take now for lowering pain for arthritis like the NSAIDS which are the non-steroidal . The side effects from SAM-e are almost none. Some people experience a little nausea. It's not a gastrointestinal effect like from NSAIDS be anti-inflammatory drugs cause of a possible effect on the brain's nausea center. It does make some people a little queasy and then we recommend that you lower your dose, and start to add to your dose so your body gets used to it. What have the clinical trials of SAM-e demonstrated? That it reduces the pain in osteoarthritis and can actually regrow, in some studies, have shown to regrow cartilage. What are SAM-e's potential side effects or health risks, short-term and long-term? They've been using it for over 20 years in Europe and there haven't been any. The only negative side effect is a little bit of nausea. It's an effect on the brain's nausea center which makes -- it's sort of like when you get off balance or get a little vertigo. It's from the brain. It's not from the stomach. It's not anything to be worried about. If you do have any kind of problems taking it, cut back a little bit and start with maybe 200 milligrams a day and then build up to whatever the dose is that's helping you relieve the pain.
How do you find your ideal dose? I like to recommend to start with 400 milligrams a day and see how that works. You can take it at once. The studies that have shown results are from 400 milligrams a day to 1200 a day. I wouldn't take more than 1600 milligrams a day. They did trials with 1600 milligrams a day and there were very few side-effects. They've done many trials to see dropout rates and people who take SAM-e and very few people drop out from the trials, meaning it's acceptable and very nonabrasive to the system, like something like an NSAID or another type of pain killer. Are there any foods that you can eat to help your body produce SAM-e? Good question. SAM-e is found in many foods. I think meats are probably the highest containing of SAM-e. What's even more important is a balanced diet, because SAM-e works best if it has folic acid available and B12 and other B vitamins. It just makes good sense to have a well balanced healthy diet and stay within your ideal body weight, because if you're walking around with a few extra pounds, it does continue the wear and tear on the joints. Does SAM-e affect people of different ages and genders differently? I don't think there was any research looking at different ages. People with milder osteoarthritis showed a greater response to SAM-e than people who were slightly older with more severe osteoarthritis. There seems to be more benefit to people before they get into the real painful stages of osteoarthritis. That's just one study and we need to study more. Arthritis does affect people 45 years and older. Two-thirds of those people are women so I think SAM-e helps anybody with osteoarthritis, but people who are more at risk are female and over age of 45. Twenty million people have osteoarthritis. Why has this been used for so long in Europe and just now coming to the U.S. market? I guess we just started getting around to testing it. We have strict rules here. However SAM-e is not a medicinal here. In Europe, it's been used as a prescription medicinal where here we can get it without a prescription. It doesn't have the same standards as a medication would have but it just took a long time for our researchers to pick up on it and for it to hit the mainstream in the U.S. Newsweek did a couple of reports and I think that's what made it come to the forefront of the American minds. How does one know that they are getting true SAM-e? Remember in the supplement industry there are no standardizations at this point. The government hasn't made requirements for supplement manufacturers to maintain. You want to get your supplements from a reputable company. When it comes to SAM-e, there's a special type. One form is 1,4-butane-disulfonate and that is the Actimet form of SAM-e, and that's the golden standard for the SAM-e dietary supplements. Actimet SAM-e is manufactured solely by BASF Pharma in Milan, Italy. If you know you're getting a SAM-e brand that has this 1,4-butane-disulfonate, you know you're getting a reliable source. The Actimet SAM-e is much more stable and less sensitive to heat and destruction. It has a shelf life of three years whereas Tosylat form of SAM-e which is not so good, breaks down within three months. I understand that it has to be taken on an empty stomach. Is that correct? Correct. The reason why it's confusing is because if they develop nausea, we recommend them starting to take it with meals. It's probably best to take on an empty stomach; however, if you develop some sort of stomach upset, take it with a light meal. Let's talk about costs. It was a little more expensive than glucosamine but there's going to be a 25% reduction because of demand. It's not cheap but it's still less than if you were to take prescription drugs. This is a great natural way to reduce pain without side effects. The average person on ongoing basis is going to be taking about 400 to 600 milligrams a day. Let's talk about mood disorders. There's other research for SAM-e for moods and also for liver damage as well. It definitely can treat mood disorders. It affects the neurotransmitters in the brain in a positive way. It's been linked to melatonin as well. Results can be achieved in a couple of weeks. If they don't get results in a couple weeks, increase the dosage or revisit their total dietary intake. What is the most exciting thing about SAM-e? It has such a great potential to regrow cartilage and up until now, what we've been doing is taking pills to cure the pain. Now there's a supplement to increase the cartilage. The difference between glucosamine and SAM-e is that glucosamine does have side effects. The SAM-e has the same if not greater benefit and a lot less potential for side effects. Can they work together? Very possible but they have to monitor themselves. Check with your doctor first. SAM-e has no current drug to drug interactions. That's a big plus. What are the primary culprits behind cartilage loss? Wear and tear. Either being overweight or exercising or overuse of an area improperly. It's possible that you are genetically predisposed to wear and tear over someone else who is not, but it has more to do with how much wear and tear you have on your joints. Why aren't more Americans aware of SAM-e? It's just been getting very popular over the last year. It's very new here. If you haven't heard of it yet, you're going to in short order. I'm a nutritionist who wrote a chapter for a prominent textbook in the nutrition world called Krause and Mahan, and I wrote the rheumatism chapter in that textbook and I wrote another chapter in the Columbia Presbyterian Medical Center's Handbook on Osteoarthritis. I wrote the food nutrition chapter for that. I have spent a lot of time in this joint health area, specifically nutrition. Not that many nutritionists are involved in arthritis.
